Interdisciplinary Comprehensive Care Approach

The Health Professionals Section is a work in progress.  We invite you to explore, connect, comment and contribute.  Interdisciplinary care may be the most useful model for the comprehensive care of patients/families with chronic illness.  What works best for the families also works for the clinician.   

While working together we learn from each other, acquire new skills, expand our repertoire and to a limited degree “cross train” in the other disciplines.  As a consequence, we more quickly refer to our colleagues in the other disciplines to develop a multi-pronged approach to patient needs. 

Enriched by this collaboration, we can better support our families and help our patients adjust to changes in their ability to be independent and enjoy life.
