MDS Financial Hardship Policy for 2022 Membership Dues

The COVID-19 pandemic has negatively affected many medical practices, it is possible that some MDS members may have a situation that justifies a dues waiver for the upcoming dues cycle.

Please fill out the following form, explaining the reason for your request. You may include any supporting documentation you feel might help inform the decision. The MDS Membership Committee Chair and Co-Chair will review these requests on a case-by-case basis. Once approved, your membership will be extended through the current year.
